Based on the findings of the National Inpatient Experience Survey 2017, Mercy University Hospital launched the “Your Hospital Journey to Home” campaign in 2018. This quality improvement initiative was developed to ensure support and planning for patients’ discharge. Through this programme, hospital staff ask patients four questions early on in their hospital journey to be able to plan a smooth and correct discharge.
These four questions serve as a reminder to staff of the importance of early discharge planning and as an information tool for patients when discussing discharge from the hospital. In order to improve the leaflet, both patients and staff were asked for their input on the questions and how to improve the campaign. The hospital also plans to monitor patient experience on a regular basis by surveying patients over the phone after they have been discharged from hospital.
